Procedure For Getting A 出生证明:

Persons applying for certificates shall make their request via in person or by mail. If applying in person, an application for the certificate is completed. The completion of this application should include:

• 出生证明上的姓名

• Relationship of applicant requesting the certificate
• Applicant’s name, address and phone number

如以邮寄方式申请, customers are required to provide the same information required as if you were applying in person along with a xerox copy of a State issued ID as listed above.

Anyone can apply for and receive uncertified copies of certificates.

Who Can Obtain Certified Copies Of Birth?

直系亲属(母亲), 父亲, 哥哥, 妹妹, 配偶, 成年子女, Grandparents - if both parents are named on the certificate, 法定代表人, Case Manager or Legal Guardian (proof required).

All adoption records are maintained by the North Carolina Vital Records Office in Raleigh. Applicants must go to the 州生命记录 Office to obtain any information and/or certificates that pertain to adoptions. No adoption records are located in the county 契约登记册 office.

州处理费- 45美元.00 (certified check/money order) -  (Make payable to: NC 州生命记录).  This includes processing and certified copy mailed to customer within three days or a letter is sent explaining why the requested change cannot be completed.
